Manage Incident Lifecycle

How to track incidents from initial report through investigation to closure.

When: As incidents are reported and progress through investigation and closure.

Navigate to Incidents.
Review the summary cards: Open Incidents, Escalated, Overdue Actions, Overdue Reporting.
For each open incident, verify the status is progressing (reported, under investigation, pending actions, closed).
Track corrective actions — ensure assignees are completing their tasks by the due date.
Follow up on overdue corrective actions immediately.
Ensure evidence is uploaded (photos, witness statements, investigation notes).
When all actions are complete and verified, close the incident.

Outcome

Incidents move through the lifecycle without stalling. Corrective actions are completed. Records are thorough.

Common Mistakes

Letting incidents sit in "reported" status for weeks. This suggests the organization is not investigating.

What Happens If Skipped

Open incidents pile up. Overdue corrective actions signal to inspectors that the organization does not follow through on safety.
